This study was to analyse the structure of the total cost, the amount of total cost and the unit cost of leprosy treatment using multidrug therapy. The study furthured to analyse the effectiveness of the drug treatment and the effectiveness of curative service. The analysis was retrospective using both primary and secondary data from Wat Mongkut Kasat Triyaram Skin Clinic. From the study, it was found that in Paucibacillary case (PB), the labour cost was the most important component in the total cost and in Multibacillary case (MB), the drug cost was the most important one. For the unit cost, it was found that the ratio of PB unit cost to MB unit cost was 1 : 7.8, the Indeterminate case had the lowest unit cost and Lepromatus had the highest unit cost. As for the effectiveness, it was found that the effectiveness of drug treatment in PB was higher than in MB. The effectiveness of curative service was lower than the effectiveness of drug treatment in both PB and MB because of the high drop out rate
